Who needs a DA Form 5960?
Any member of a uniformed service who is entitled to basic allowance for quarters and/or to a variable housing allowance needs a DA Form 5960.

The form is used for the authorization to start, adjust, or terminate basic allowance for quarters and/or variable housing allowance.
Is DA Form 5960 accompanied by other forms?
A DD Form 137 may be required.
When is DA Form 5960 due?
Soldiers must certify their dependency status every three years. Soldiers with secondary dependents must recertify annually.
How do I fill out DA Form 5960?
Blocks 1-3 are for your basic identifying information.
In Block 4, mark with an “X” the type of action.
In Blocks 5-6, enter your unit address and the effective date of the action.
The applicable BAD type should be marked with an “X” in Block 7.
In Block 8 “Marital/Dependency Status”, mark the appropriate selection, follow on to any necessary sub-blocks.
In Block 9, check “Adequate”, if you have been assigned to government quarters, check “Not available” if you are residing off post, or “Transient” if you are living in transient facilities.
If you have dependents, list all of them in Block 10.
Checking fields in Block 11, you certify the support of your dependents.
Block 12 requires accounting information of your monthly expenses and the monthly expenses of any dependents not currently living with you. Indicate your current mortgage, insurance or other monthly obligations. If you are leasing quarters, you must include the contact information for the landlord and the address of the location.
You and your commanding officer must sign and date the form in Blocks 13-16.
Where do I send DA Form 5960?
Depending on the circumstances, you must provide the form to the Installation Finance Office or to the USER Pay Center (UPC).
